An ACL tear can end your sports season — but with modern arthroscopic surgery, most athletes return to their sport within 6–9 months.

The Anterior Cruciate Ligament (ACL) is one of the four major ligaments in the knee and is critical for knee stability, especially during twisting and jumping movements. ACL tears are among the most common serious sports injuries.

How ACL Tears Happen

Most ACL tears are non-contact injuries — they occur when the athlete suddenly changes direction, decelerates, or lands awkwardly from a jump. Common sports include football, cricket, basketball, kabaddi, and badminton.

Typical symptoms: - A loud "pop" at the time of injury - Immediate swelling of the knee - Severe pain preventing continued activity - Feeling of instability or "giving way"

Diagnosis

Clinical examination by an experienced orthopaedic surgeon can usually identify an ACL tear. The Lachman test and pivot-shift test are highly accurate. MRI scan is used to confirm the diagnosis and identify associated injuries (meniscus tears are common with ACL tears).

Treatment

Non-Surgical For older, less active patients or those with partial tears, rehabilitation with physiotherapy and a knee brace may be sufficient.

Arthroscopic ACL Reconstruction For athletes and active individuals, surgery provides the most reliable return to sport. At Shivneri Hospital, ACL reconstruction is performed **arthroscopically (key-hole surgery)** — using tiny incisions, a camera, and specialised instruments.

The procedure: The torn ACL is replaced with a graft, usually taken from your own hamstring tendon. The graft is fixed in the exact position of the original ACL.

Recovery & Return to Sport

  • 0–2 weeks: Crutches, ice, elevation, range of motion exercises
  • 2–6 weeks: Progressive weight-bearing, strengthening begins
  • 3–4 months: Running starts
  • 6–9 months: Return to full competitive sport
  • 12 months: Optimal strength and confidence for high-level sport

Early surgery followed by dedicated rehabilitation gives the best outcomes. Don't ignore ACL tears — an unstable knee leads to further damage including meniscus and cartilage injury over time.
