Heel Pain: Causes, Symptoms and Modern Treatment Options

Back to Blog

Heel pain is one of the most common orthopaedic complaints. Learn about plantar fasciitis, calcaneal spurs, and when to see a specialist.

Heel pain is one of the most common reasons patients visit an orthopaedic surgeon. It affects people of all ages and can significantly impact daily activities, work, and quality of life.

Common Causes of Heel Pain

Plantar Fasciitis is the most frequent cause, accounting for approximately 80% of all heel pain cases. The plantar fascia is a thick band of tissue that runs from your heel bone to your toes. When this tissue becomes inflamed, it causes sharp stabbing pain โ€” typically worst with the first steps in the morning.

Calcaneal Spurs are bony outgrowths that form on the underside of the heel bone. They often develop alongside plantar fasciitis and are visible on X-ray.

Achilles Tendinitis causes pain at the back of the heel where the Achilles tendon attaches. Common in runners and active individuals.

Heel Fat Pad Syndrome occurs when the protective fat pad under the heel thins or becomes damaged, causing pain with weight-bearing.

Stress Fractures of the calcaneum (heel bone) can occur with repetitive impact activities or in patients with osteoporosis.

When to See a Doctor

You should consult Dr. Bhardwaj if: - Pain persists for more than 2โ€“3 weeks despite rest - Pain is severe or waking you from sleep - You notice swelling, redness or warmth - Pain is getting progressively worse - You have diabetes or poor circulation

Modern Treatment Options

Non-Surgical (First Line) - **Physiotherapy and stretching exercises** โ€” calf stretches and plantar fascia stretches are highly effective - **Custom orthotics** โ€” professionally fitted shoe inserts that redistribute pressure - **NSAIDs** โ€” anti-inflammatory medications for pain relief - **Corticosteroid injections** โ€” targeted injection to reduce inflammation - **PRP (Platelet-Rich Plasma) therapy** โ€” uses growth factors from your own blood to promote healing

Surgical (If Conservative Fails) Surgery is rarely needed โ€” only in persistent cases unresponsive to 6+ months of conservative treatment. Options include plantar fascia release and calcaneal spur removal using minimally invasive techniques.
